What you'll do 

  • Provide individualized care to residents according to each resident’s plan of care. 

  • Assist residents with activities of daily living (ALDs) including bathing, grooming, and dressing. 

  • Assist with mealtime activities, including feeding residents as needed.    

  • Assist residents with mobility, including transferring and positioning them safely.  

  • Monitor residents’ health status and vital signs and report any changes or concerns to the nurse.  

  • Engage residents in social and recreational activities to promote overall well-being.  

  • Maintain safe, clean, and organized living environment for residents.  

  • Perform other duties as required to provide exceptional care to each resident.  

What you need 

  • Active certification as a Certified Nursing Assistant in state of practice.

What you'll do 

  • Supervise, coordinate, and direct the work of nursing assistants (CNAs). 

  • Provide individualized nursing care to residents according to each resident’s plan of care.  

  • Assist in the admission, transfer, and discharge of patients.  

  • Administers prescribed medications and treatments as ordered by the physician. 

  • Explains procedures and treatments ordered to gain patients’ cooperation and allay apprehension. 

  • Observe vital signs to detect deviations from normal and gage progress of the patient. 

  • Maintains patients’ medical record on nursing observation and action taken, such as medications and treatments given, reactions, tests, intake, emission of liquids and solids, and vital signs. 

  • Records nursing needs of patient on care plan to assure continuity of care. 

  • Observes emotional stability of patients, expresses interest in their progress, and prepares them for continuing care after discharge. 

  • Perform other duties as required to provide exceptional care to each resident.

Director of Admissions

As the Director of Admissions, you will play a crucial role in upholding our commitment to Care Beyond Compare by effectively managing and maintaining our census. Your primary responsibility will be to support patients and their families during the transition from acute care to post-acute care, providing guidance and assistance throughout the process. This role demands excellent communication and interpersonal skills, as you will serve as the main point of contact between the healthcare team, patients, and their families. You will evaluate potential admissions, collaborate with various departments to ensure a smooth admission process, and implement strategies to attract and retain patients. The ideal candidate will possess a compassionate approach, strong organizational skills, and a deep understanding of the healthcare continuum.

What you'll do 

  • Develop and implement strategic plans to achieve and maintain census targets.  

  • Monitor and analyze census trends and develop plan of action to address census challenges.  

  • Identify potential referral sources and build strong relationships with hospitals, physicians, community organizations, and other healthcare providers.  

  • Represent the facility at community events, health fairs, and other outreach events to promote the facility and its services.  

  • Collaborate with the interdisciplinary team to ensure the facility can meet the needs of incoming patients.  

  • Conduct facility tours for prospective patients and their families, providing information about services, amenities, and care options.  

  • Other duties as necessary for the successful operation of the facility.  

 

What you need 

  • Bachelor’s degree in healthcare administration, marketing, or related field preferred.  

  • Two years of experience in admissions, marketing, or sales within a healthcare setting, preferably in a skilled nursing facility.  

  • Strong understanding of the skilled nursing and long-term care industry.  

  • Proven ability to build and maintain relationships with referral sources and community partners. 

  • Excellent communication and interpersonal skills.  

  • Highly organized and detail oriented.  

  • Ability to work independently or as part of a team.
